All you need is the editor, cartographer and interior artist. Mike D. might be able to help you there.
As for dimensions, I would say "standard" with other modules from this time frame. As for weight - grab another 32 page module and poster map and weigh it. (I don't mean to be inexact, but that is how I've managed to get through many D&D items I didn't have).
As for Collector's Arcana - note the poster map and that RS1 was inadvetently assigned to Van Richten's Guide to the Lich. The Richten Lich Guide should probably be RR6 as that is in keeping with the other guides and there is no RR6.

Got me there, I was actually just listing Parkinson for another item, so I must have had him on my brain! Caldwell's signature logo is apparent as always . . .
